Details

The SON form is to be used to report all Air Safety significant occurrence notifications in accordance with RA 1410.

Note: This form has been updated to replace the legacy Initial Report.

A SON must be raised by the relevant Aviation Duty Holder (ADH), Accountable Manager (AM), Delivery Team (DT)/Type Airworthiness Authority (TAA) or unit as appropriate, in order to bring noteworthy occurrences to the attention of the Ministry of Defence (MOD), Permanent Joint Headquarters (PJHQ) and Director General Defence Safety Authority (DG DSA).

SONs may include, but are not limited to, those occurrences where an accident has or could have occurred, or which could be deemed sensitive (eg VIP involved; possible societal, media or ministerial interest). Occasions requiring a SON are summarised in RA 1410 Annex A.

SON follow up action, such as a return to flying, will be recorded within the associated DASOR, or where there is no DASOR by separate notification to original addressees.

SONs are to be passed to:

MOD Deputy Chief of Defence Staff Duty Officer (DCDS DO) on Mil: 9621 88938 or Civ: 030 6788 8938 for occurrences not in an Operational Theatre.

Permanent Joint HQ Duty Ops Controller (PJHQ DOC) on Mil: 9360 55310 or Civ: 01923 955311 for occurrences in an Operational Theatre.
