Use this form to apply for replacement HGV plating and testing documents if the originals have been lost, stolen or damaged.
Include any damaged documents with your application. If you find any lost documents later, send them back to DVSA.
DVSA will contact the email you provide on the form to take your payment and send your certificate. Your application may be delayed if you do not provide a valid email address.
