  1. The Eder Sands case has been removed from the current investigations table following publication of the report and safety flyer. A link to a safety bulletin for the Honeybourne III case has been added.

  2. The table has been updated with a link to an interim report on the investigation of a fatal fall from height while boarding cargo vessel Finnhawk from the pilot vessel PV Humber Saturn.

  3. Three cases have been added to the current investigations table. The grounding and loss of fishing vessel Sustain at Loch Broom, Scotland in November and last week, the man overboard, presumed deceased, from fishing vessel Amadeus in the German Bight are under investigation. A preliminary assessment also started last week into the fatal man overboard from the 8.18m fishing vessel Nista off Luing, Scotland.

  4. The Seadogz investigation has been removed from the table following the publication of our report. The table entry for the fatal sinking of a migrant boat on 14 December 2022 has updated with a link to an interim report.

  5. The Resurgam investigation has been removed from the table following publication of the report and safety flyer. The status for Eder Sands has been updated to consultation stage.

  6. Our current investigations table has been updated with links to interim reports for the Awesome and Ali Ka cases.

  7. Our current investigations table has been updated following the loss of propulsion of the UK registered passenger vessel Spirit of Discovery in the Bay of Biscay on 4 November, resulting in multiple passenger injuries during heavy weather.

  8. Following today's report publication, the migrant boat with loss of at least 27 lives on 24/11/21, has been removed from our current investigations list.

  9. Following today's report publication, the RRS Sir David Attenborough case has been removed from our current investigations table. An ongoing investigation has also been added after a recreational diver was struck and fatally injured by Karin, a passing dive support vessel, in Scapa Flow, the Orkney Islands, Scotland.

  10. The current investigation table has been updated following yesterdays fatal collision between the Isle of Man registered general cargo vessel Verity and the Bahamas registered bulk carrier Polesie in the German Bight traffic separation scheme. The collision resulted in the sinking of Verity with 1 person declared deceased. Two of the remaining 6 crew members were recovered from the water and 4 are missing, with searches ongoing. This investigation is on behalf of the Isle of Man Ship Registry, in accordance with the MoU between the MAIB and Red Ensign Group Category 1 Registries of the Isle of Man, Cayman Islands, Bermuda and Gibraltar.

  11. The status for the investigation into the fatal sinking of an unnamed migrant boat on 24 November 2021 has been updated. It is intended that the investigation report will publish on 9/11/23. The status for the fatal accident on board the RIB Seadogz has also been changed to consultation stage.

  12. We started two investigations into accidents resulting in a fatality on UK registered vessels last week - one involved a fall from the sail training vessel Pelican of London's gangway at Sharpness, Gloucester and the other resulted from a fishing gear failure on board FV Honeybourne III off Beachy Head, East Sussex. Both cases are now on our current investigations list.

  13. An investigation has been added to our current investigations list following the capsize of fishing vessel Lexi Rose (BF370) off Macduff, Aberdeenshire, Scotland with loss of 1 life on 21 September 2023.

  14. A propulsion failure on the UK registered passenger vessel Ocean Diva (9938078), which then made contact with a moored barge at Crossness, Halfway Reach on the River Thames, London on 22 June, has been added to our current investigations table.

  15. The main engine shutdown of the Malta registered ro-ro cargo ferry Mazarine and the subsequent breach of its underwater hull on Wolf Rock off Lands End, Cornwall, England, has been added to the list of investigations being led by other administrations. Malta is investigating, with the UK declared as a substantially interested State.

  16. Our investigation table has been updated to remove the Scot Carrier/Karin Hj case following publication of our investigation report and to add an investigation into the total constructive loss of the commercial swim event support vessel, Channel Queen.
