Use this form to apply for an accessibility type approval certificate for a bus or coach to show it meets wheelchair accessibility, general accessibility or wheelchair and general accessibility rules.
You also need to fill in a accessibility requirements specification form (PSVA 12) to support your application.

Save the application form on your computer.
Send the application form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for an accessibility certificate.
Pay the application fee by credit or debit card (DVSA will tell you how to do this after youve applied), or by cheque or using a pre-funded account.
