Details

The leaflet is shown to patients prior to their discharge from hospital to go to further non-acute bedded care, such as a care home.

The editable PDF allows specific contact details to be added in case patients need to speak to someone after leaving hospital.

  1. Added translations in Arabic, Bangla, Chinese, Traditional Chinese, French, Gujarati, Polish, Portuguese, Punjabi Gurmukhi, Spanish, Urdu and an easy read version of the leaflet in English.

  2. Updated to reflect changes to hospital discharge guidance following the end of the national discharge fund.

  3. Updated to add a call out to reflect the leaflet is out of date and should not be used; deleted the PDF, PowerPoint, easy read and translated versions of the leaflet. An updated leaflet will be published soon.

  4. The language in the translations has been updated in line with the new leaflet.

  5. Added translations and accessible versions of leaflets.

  6. First published.
