Details

Use this form to apply for IVA for lorries and goods vehicles with a gross vehicle weight of over 3,500kg.

You need to say if you can have the IVA test done by a video call rather than in person when you fill in the form.

  1. Download and fill in the application form.

  2. Save the application form on your computer.

  3. Send the application form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for IVA.

  4. Pay the application fee by credit or debit card (DVSA will tell you how to do this after youve applied), or using a pre-funded account.

Your application can be delayed if you do not fill in the application form correctly.
