The accident investigation report by the Republic of the Marshall Islands Maritime Administrator into the mooring failure of Valaris DS-4 at Hunterston Jetty on the Clyde Estuary, Scotland during storm force winds on 2 February 2021 (link to external website) was published on 1 August 2022.

The Republic of the Marshall Islands Maritime Administrator earlier issued Marine Safety Advisory No. 14-21 on 5 October 2021.
