The report into MAIBs investigation of a chief officers fall from a hatch cover while loading acargo of privately owned motor yachts onto the general cargo ship Norjan at Southampton, UK is now published.
The accident on 18 June 2014 resulted in multiple fractures to both of the chief officers ankles and he was unable to return to work for 12 months.
The report contains details of what happened, actions taken and recommendations.
