These should only used in limited circumstances where you do not have access to the eForm portal, such as where you only hold a civil contract and are applying in relation to civil contempt or breach of a civil injunction under the Anti-social Behaviour, Crime and Policing Act 2014

In the event that systems are unavailable for an extended period, the LAA will advise accordingly. This could potentially include using paper forms for any lengthy system outage.

Depending on the applicants answers the CRM14 will tell you whether to complete a CRM15.
