Details

Use this form to apply for an HGV to have its first MOT (annual test) if its either:

  • registered with DVLA and now needs a test (or plating and testing) after it was previously exempt from being tested
  • not registered with DVLA and was manufactured more than 25 years ago

Theres a different process to book an MOT for a:

  • registered vehicle that already has plating documents.
  • non-registered vehicle thats being approved and issued with plating documents in the Individual Vehicle Approval (IVA) scheme before its registered

Send the form online

  1. Download and fill in the application form.

  2. Save the application form on your computer.

  3. Send the application form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for a vehicle test for a lorry.

  4. Pay the application fee by credit or debit card (DVSA will contact you to do this after youve applied) or using a pre-funded account.

Send the form by post

You can also send the application by post. You can also pay by cheque or postal order when you apply by post.
