This form is for disputes in the fast track, intermediate track or multi-track. A court will send the form to the claimant or defendant if they are not using a legal representative.

The directions questionnaire now covers intermediate track. The intermediate track option has now been added to the 'Allocation to track' section. Guidance on the use of the Welsh language when filing witness evidence has been added to the 'Witnesses' section.
